Office Payment Policy
- Examinations & office visits are payable at time of visit.
- Glasses and contact lenses require a 50% deposit when ordered.
- Glasses and contact lenses are payable in full when received.
- We accept MasterCard and Visa.
- Insurance Forms-We are participating providers for Blue Cross,
BlueShield, Medicare and Medicaid, and other insurance's.
Our staff will gladly complete your insurance forms for additional
insurance carriers to assist you in receiving reimbursement
for our services.
